QR Code Design and Customization
A QR code tolerates a surprising amount of decoration and it is not unlimited. Colour, a logo, rounded modules and a small print size all draw on the same budget, and the codes that fail in print are almost always the ones that spent it three times.
Every tool here measures its own effect rather than warning you in the abstract, and none of them will let you download a code that did not read back.
Colour and contrast
Contrast is the measurement that governs everything else. Below about 3:1 nothing scans.

One Budget, Four Ways to Spend It
A scanner needs to tell dark modules from light ones, find the three corner patterns, and sample cleanly inside each module. Everything on this page makes one of those three jobs harder.
Contrast below roughly 3:1 stops the first job outright. A logo deletes modules and relies on error correction to rebuild them. Rounded modules and dots remove inked area from every module. Printing small removes margin from all of it at once. Any one of these is usually fine. Three at once is where codes stop working, and the reason is arithmetic rather than bad luck.
What Stays Fixed Whatever You Do
Two things are not design decisions. The quiet zone, the blank margin of four modules on every side, is part of the symbol and cropping it is the most common way a finished code is broken by a layout. And the three finder patterns stay square in every module shape offered here, because their edge ratios are what a scanner measures the code's position from.
